Clinic Appointments

The mission of DOH–Leon’s Immunization Program is to protect the health and safety of all citizens and visitors in our community by preventing vaccine-preventable diseases.

Immunizations

Our Services 

  • Administer free immunizations to children from birth through 18 years of age including
    • Routine recommended immunizations (“baby shots”) for children from birth through 2 years of age
    • Required immunizations for daycare and school attendance, including immunizations needed for kindergarten and seventh grade entry.  Immunization record for daycare and school attendance is provided on a Florida Certification of Immunization, DH 680 form, commonly called “the blue card.”
  • Administer immunizations for a fee to adults 19 years of age and older
  • Provide education to individuals and groups regarding immunizations
  • Provide reminder notification to parents when children immunized through the health department are due, or overdue, for immunizations, especially for children less than 2 years of age
  • Provide guidance and support to local medical providers on immunization-related issues including immunization guidelines and vaccine storage and handling issues
  • Provide guidance to public and private school personnel regarding immunizations needed for school entry or attendance
  • Update and maintain the computer database containing immunization records for all immunizations administered at DOH-Leon to both children and adults
