Allergies – If your child has an allergy that may require injectable epinephrine or an oral antihistamine to be given if exposed to that substance (e.g. food, insects, etc.) please have your child’s healthcare provider complete an Individualized Allergy Action Plan. If your child will need to carry his/her emergency injectable epinephrine or inhaler, please have the healthcare provider complete an Authorization to Carry. For each prescribed medication, please complete a separate Medication Permission Form. As the parent or guardian, please also complete a Consent to Share.
