Asthma – If your child has asthma and may need to use their rescue inhaler before PE or recess, or may need assistance with their rescue inhaler, you will need to have your child’s healthcare provider complete an Individualized Asthma Action Plan. You will need to complete a Medication Permission Form and a Consent To Share. If your child will need to carry his or her inhaler during the day, or back and forth to school, please also have your child’s healthcare provider complete an Authorization To Carry.
