Application for Florida Birth Record

The Vital Statistics Office provides birth and death certificates for Florida only. The staff reviews certificates for accuracy and completion and provides assistance with the preparation and filing of vital records to funeral directors, medical examiners, and hospital personnel. The Vital Statistics Office does not process marriage licenses, divorce records, probate, wills, or social security cards.

All birth certificates for births prior to 1917 must be requested from the State Office of Vital Statistics in Jacksonville. For more information, call 904-359-6900.

Requirements

Birth certificates can be issued only to the registrant if of legal age (18 or older) or to parents listed on the birth record. Legal guardians must provide guardianship paperwork. Legal representatives of one of the above persons or other individuals must provide a recorded or certified copy of a court order.
