Creating a Healthy Community Together

Tallahassee– What are the primary health issues facing some Leon County residents? As part of the
Community Health Assessment (CHA) process, a series of meetings in five target areas will begin
May 14. During these meetings residents will hear results of the previously conducted health survey,
discuss additional health concerns, set health priorities, and identify ways to improve health in their
community/neighborhood. All residents are invited to attend and share their thoughts and opinions on
health and quality of life in our community. Children are welcome and refreshments will be available.


“This will be a great opportunity for community residents to let us know what is important to them when it
comes to health, “ said Claudia Blackburn, Health Officer at DOH-Leon. “The information gathered at
these meetings is an integral part of the comprehensive health assessment that is currently underway”.


In an effort to gather additional input from neighborhoods surveyed, community meetings are scheduled
in five areas of Leon County:

May 14, Greater Frenchtown at Dade Street Community Center from 12:30pm to 2:30pm

May 21, Fairbanks Ferry Road Area at China Hill Primitive Baptist Church from 3:00pm to 5:00pm

  • 458 Fairbanks Ferry Road, Tallahassee, FL 32312

June 11, Macon Community, Terrance Freely Community Center from 12:30pm to 2:30pm

  • 2940 Grady Road, Tallahassee, FL 32312

June 18, Southside Community, Smith-Williams Service Center from 1:00pm to 3:00pm

  • 2295 Pasco St, Tallahassee, FL 32310

June 25, Hwy 20/Aenon Church Road Area, Berean Christian Fellowship from 12:30pm to 2:30pm

  • 6214 Blountstown Highway, Tallahassee, FL 32310
Attendees may share their thoughts, opinions, and concerns surrounding the following questions:
   1. Which of the issues and concerns noted by survey respondents did you agree with?
   2. Are there other issues and concerns you think are missing that you think are important to
       mention? If so, what are they?
   3. What seems to be the most critical health issues or concerns for the neighborhood or
       community?

Children activities will be available for meeting attendees at locations specified above. For more
