DOH-Leon To Relocate Birth, Death Certificates Service to R&S Clinic

New location opens September 4

Tallahassee – The Florida Department of Health in Leon County will relocate its Vital Statistics Office to the Roberts & Stevens Clinic at 1515 Old Bainbridge Road next month.

Starting September 4, anyone who’d like to purchase a birth or death certificate through DOH-Leon can do so at the R&S Clinic.

Hours will be 8 a.m.-5 p.m. Monday thru Thursday and 8:30 a.m.-5:00 p.m. Friday.

Birth certificates are available from 1917 to the present for births in the state of Florida. Cost is $12 per copy.

Death certificates are available from 2009 to the present for deaths in the state of Florida. Cost is $10 per copy.

Birth certificates can also be purchased through the Leon County Tax Collector’s Office at the following locations. Cost is $18.25 per copy. For more information, call the LCTCO at (850) 606-4700.

Birth certificates can be requested by the mother or father of the child, the child himself/herself after age 18 or the legal guardian of the child with proper documentation and identification.

Death certificates can be requested by a licensed funeral director or the family of the deceased.

In both cases, acceptable identification must be presented. This includes a valid driver’s license, a state identification card (issued by the Division of Motor Vehicles), passport or military ID.
