Florida death certificates are issued through the Bureau of Vital Statistics at the Department of Health. Orders may be placed online, by mail, or in-person.


Obtain a Copy in-Person or by Mail

Death Certificates are available for deaths occurring in the state of Florida from 2009 to present.

  • A completed Death Certificate Application should be submitted along with the appropriate fee.
  • Visit (or mail your request to) our main office. We are open Monday through Friday from 8 a.m. to 4:30 p.m.
  • A legible copy of a valid driver’s license, passport, state-issued ID, or military identification card must accompany all requests.
  • You must be a family member or legal representative when requesting cause of death.

Any death prior to 2009 must be requested from the Florida Department of Health’s Bureau of Vital Statistics in Jacksonville or through VitalChek®.
