Back-to-School Immunizations

The Florida Department of Health in Leon County will be offering back-to-school immunization services at:

June

  • 6/17, 6/18, 6/24, and 6/25 from 8 a.m. - 4 p.m.
  • Location: 872 W. Orange Ave, Tallahassee, FL 32310

July

  • 7/1, 7/2, 7/8, and 7/9 from 8 a.m. - 3:30 p.m.
  • 7/22, 7/23, 7/29, 7/30, and 7/31 from 8 a.m. - 4 p.m.
  • Location: 1515 Old Bainbridge Road, Tallahassee, FL

August

  • 8/5 and 8/6 from 8 a.m. - 4 p.m. at 1515 Old Bainbridge Road, Tallahassee, FL
  • 8/9 from 8:30 a.m. - 12:30 p.m. at Lively Tech - 500 Appleyard Dr, Tallahassee, FL
  • 8/11 - 8/14 from 8 a.m. to 4 p.m. at 1515 Old Bainbridge Road, Tallahassee, FL

A parent or legal guardian must accompany minors. Please bring:

  • Your child's most recent immunization record (Form DH 680)
  • Your government issued ID 
  • Insurance card (if applicable)
  • An administration fee of $24 may apply.

No child will be denied services based on inability to pay.

Childhood Immunizations

DOH-Leon offers childhood immunizations that are required for school and child care. Please note that all vaccinations are offered on an appointment-only basis.

Required immunizations are provided free of charge to all eligible children 0-18 years of age (funded by the federal Vaccines for Children Program.) A child qualifies for free vaccinations if one of the following criteria is met:

  • Child is enrolled in Medicaid, Medipass, or Medicaid HMOs.
  • Child has no health insurance.
  • Child’s health insurance does not cover immunizations.
  • Child is American Indian or Alaskan Native.

The primary goal of our immunization program is to prevent the spread of vaccine-preventable diseases in Leon County. Early immunizations (also called vaccines) are especially important because young children lack fully developed immune systems and are the most vulnerable to disease. Immunizations are a safe, effective way to protect your child from getting measles, mumps, and many other serious diseases. 

Parents and guardians: Please bring your child's most recent immunization record and your government issued ID with a recognizable photo, date of birth, and signature.

Florida SHOTS

Florida SHOTS (State Health Online Tracking System) is a free, statewide, centralized online immunization registry that helps parents, health care providers, and schools keep track of immunization records. Ask your health care provider for a personal identification number so you can view your child’s immunization history and print a copy of your child’s Florida Certification of Immunization (DH Form 680).
