Adult Immunizations

Clinic Appointments

Adults are also at risk for contracting vaccine-preventable diseases. To protect vulnerable populations from becoming infected, the Vaccines for Adults Program (VFA) in Florida provides vaccines to our adult population to prevent infection with one of these potentially devastating diseases.

To be eligible to receive VFA vaccine, one must be an:

  • Uninsured adult aged 19 years and older
  • Underinsured adult aged 19 years and older (i.e., health insurance does not cover the cost of the vaccine to be administered)

The clinic also offers vaccination services to adults who are insured or intend to self-pay.

Appointments

Immunizations are offered on an appointment-only basis. To schedule an appointment, please call 850-404-6403.

Community members are advised to follow CDC immunization schedules based upon the appropriate age group for when the dosage is most effective.

Check with your insurance provider to see if vaccines are covered under your insurance plan and where to access vaccines through the plan. Anyone age 19 and older may qualify for another free limited selection vaccine program, otherwise, there is a fee for vaccines. 


Fees

DOH-Leon charges an $18.00 administration fee for the first vaccine (in addition to the vaccine cost) and a $16.50 administration fee for any same-day additional vaccines (in addition to the vaccine cost) for all adult immunization service visits. Pricing is subject to change due to fluctuations in vaccine costs. See current DOH-Leon immunizations fee schedule here.

  • Download, print and complete these forms for each individual receiving services and have them ready for your appointment:
  • Bring your photo ID and all previous immunization records for each individual needing services. Prior vaccination records must be available prior to or at visit time.
